Passed away on 4 Dec. 2009 at 3.50pm
Cortege leaves form residence at No. 11, Lorong TAmbun Indah 7, Taman Tambun Indah, Simpang Ampat, 14100 S.P.S on Tuesday , 8 Dec. 2009 at 10.30am and thereafter for burial at Prestavest memorial Park, Taiping, Perak.
